Job Description

Case Manager

Location: Albany, CA
Schedule: Monday–Friday, 9:00 AM to 5:00 PM
Pay Range: $100K to $120K annually

We’re hiring a Case Manager to help individuals move forward by connecting them with the services and care they need. You’ll work in an office-based role supporting people who face complex challenges—whether that involves housing, behavioral health, or medical care—and play a key part in improving long-term outcomes.

Key Responsibilities:

  • Build supportive relationships with clients and families
  • Identify appropriate services and assist with access and coordination
  • Keep accurate, up-to-date case files
  • Follow up regularly to ensure continued engagement and progress
  • Work alongside internal staff and external providers

Requirements

  • Bachelor’s degree in Social Work, Psychology, Nursing, or a related field (Master’s preferred)
  • At least 2 years of case management or client coordination experience
  • Strong communication and organization skills
  • Familiarity with local support systems and services
  • Licensure (e.g., LCSW, LMSW, RN) is a plus
