The Enrollment Case Manager facilitates the eligibility determination process for Home and Community ****d Services (HCBS) applicants. This role involves conducting in-home intake visits, assisting with application submissions, and helping applicants select appropriate providers. The Case Manager also builds relationships with community organizations and stakeholders to improve the application process. Travel throughout the assigned county is required.

Responsibilities:
- Travels throughout the assigned county and neighboring counties as dictated by business needs, working primarily in applicants' places of residence.
- Conducts intake visits for HCBS applicants and acts as a facilitator for the eligibility determination process.
- Presents and discusses the concept of HCBS programs, and advises applicants of their rights and responsibilities.
- Assists with application submission, including data entry of the Compass Application.
- Assists applicants with selecting an MCO, Life Provider or Support Coordinators that best meets their needs.
- Assists with developing relationships with community-based organizations, advocates and stakeholders.
- Fosters and maintains relationships with parties serving our mutual consumers, including physicians, County Assistance Offices, providers, nursing homes and rehabilitation facilities.
- Facilitates the efficiency of the application process.
- Attends regularly scheduled meetings.
- Reports on schedule availability at regular intervals.
- Occasionally lifts, carries, or otherwise moves items weighing up to 25 pounds.
- Performs other duties as assigned by management.
Required Skills & Certifications:
- A minimum of one year experience in public or private social work; or a minimum of two years of case work experience including one year of experience performing assessments of individuals functional ability to determine the need for institutional or community-based services; and a Bachelor's degree, with or supplemented by a minimum of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science; Or a Bachelor's degree with a social welfare major; Or any equivalent combination of experience or training with successful completion of a minimum of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science.
- Excellent organizational, written and verbal communication skills.
- Ability to interact courteously and effectively with a variety of audiences, including the disability community.
- Ability to handle pressure and perform multiple tasks.
- Ability to work independently.
- Experience entering data into automated information systems.
- Ability to maintain cultural sensitivity.
- Must have their own car.
