Responsibilities

Case Manager (Master)

Evaluates (assesses), plans, implements, coordinates, and monitors the options and services to help fulfill the person’s health needs, using the available resources to promote quality and cost-effective results. Advocates for the member and serves as liaison with the community and governmental agencies and other stakeholders. Establishes the case management plan with member. The plan is discussed with the Interdisciplinary Care Team (ICT)and with the clinical supervisor. Takes into consideration the member’s strengths to prepare the case management plan. Involves the family, caregivers, and member in the case management process. Makes sure that the member has access to all the available tools for his or her rehabilitation and for a better quality of life

Responsibilities and essential duties:

  1. Apply case management program criteria to identify individuals appropriate for case management services.
  2. Assess and identify issues where case management services have potential for making a positive impact toward achieving identified goals and desired health outcome.
  3. Informs the member about the services provided by the Case Management Program. Facilitates informed options and decision-making. Explains the member the consent to receive program services, to disclose and obtain information, the complaint and grievance process, and any other related information.
  4. Use a collaborative approach with the Quality, Financial, Pharmacy, Utilization, Access Center departments, among other stakeholders to coordinate services and track utilization and progress toward recovery.
  5. Conduct a throughout assessment using the biopsychosocial model.
  6. Promotes the recovery and well-being of the member incorporating in the provision of case management services the development of case management plan with practices that are supported by evidence-based clinical guidelines and standards of case management (e.g., SMART model, Cognitive Behavioral Therapy, Motivational Enhancement Theory, among others).
  7. Post-discharge follow-up phone calls after psychiatric hospitalizations.
  8. Asist member with a safe transition of care, from one setting to the next.
  9. Conduct assessment and re-assessments, including the administration of PHQ9, GAD 7, suicide risk assessment, and other screening tools.
  10. Complies and promotes the established confidentiality regulations.
  11. Documents the clinical information on time in the appropriate platforms.
  12. Prepares monthly reports establish by organization based in our clients’ needs.
  13. Visits the members at home or at another accessible place, as necessary.
  14. Coordinates and visits the community agencies or practitioners in accordance with the member’s case management services need.
  15. Participates in case discussion with mental health providers and other community or governmental agencies professionals.
  16. Establishes a clear and concise individualized case management plan with the member and family/caregiver.
  17. Monitors the individualized case management plan, measures progress, and establishes the discharge date or continuation of services if necessary.
  18. Advises the person, the family, and significant others of the options availed for their recovery and self-management tools.
  19. Completes the visit registration log sheet for each intervention conducted.
  20. In compliance with the policies, procedures, and code of ethics maintains a therapeutic relationship with the family or support system.
  21. Serve as a liaison between member’s family, mental health providers and physical health providers.
  22. Analyses and discuss cases with the psychiatric hospitals’ clinical personnel.
  23. Practices cultural competency, conscientiously, and with respect for diversity.
  24. Implements models of collaborative practice to include therapists in the mental health field and the primary doctor, including the four-quadrant model.
  25. Promotes and incorporates the use of evidence-based practices and empowerment strategies with an emphasis on prevention of exacerbations and complications.
  26. Maintains an adequate case load.
  27. Represents the program or the corporation in interdepartmental meetings, with government agencies, commercial accounts, IPAs, for profit and nonprofit organizations, among others.
  28. Other tasks assigned by the supervisor.
  29. Participate in the ICT meetings.

Qualifications

Minimum qualifications:

  • Master’s in social work, Psychology, Psychological Counseling, Mental Health Counseling, or Psychiatric Nursing
  • Valid and current active license for Social Work, Psychiatric Nursing, Psychological Counseling, Psychology, or Mental Health Counseling.
  • Valid membership for Social Work and Psychiatric Nursing (colegiacion)
  • Comply with the continuing education hours as they correspond to their specialty
  • Relevant Work Experience: Complete a supervised two or more years of field experience in case management, health, behavioral health, or vocational rehabilitation as part of the degree.
  • Available to travel throughout Puerto Rico
  • Basis knowledge of psychotropic agents required
  • Knowledge in Microsoft office, Outlook
  • Preferable fully bilingual (Spanish and English)
